Medication therapy management is a distinct service or set of services that improve the therapeutic results of the individual patient.
Medication therapy management includes a wide range of activities and professional responsibilities within the licensed pharmacist, or other qualified healthcare provider, and scope of practice. These services include, but are not limited to the following, according to the individual needs of the patient
- Perform or obtain the necessary assessments of health status of the patient;
- The formulation of a treatment plan of medication;
- Selection, initiate, modify or manage medication therapy;
- Monitoring and evaluation of response to therapy, including safety and effectiveness;
- Performing a comprehensive medication review to identify, resolve and prevent problems Medication, including adverse effects;
- Documenting the care delivered and communicating essential information to other providers of primary care to patients;
- Provide oral education and training designed to improve patient understanding and appropriate use of his / her medications;
- Provide information, support services and resources designed to improve patient adherence to their / its therapeutic regimens;
